For example, If these situations apply to you, signing up to a HOPE membership Program will highly benefit you: 

• You have a high-deductible health insurance policy and might otherwise pay hundreds or thousands of dollars in out-of-pocket costs in a year. 
• You have a chronic health condition that requires constant monitoring, like diabetes or heart disease. 

• You’re elderly and likely to frequently visit a primary care doctor. 
• Your employer offers membership medicine and covers all or part of the fee.

we have designed different membership plans to ensure that we can cover you, your loved ones, or your employees: 
 

• Individual Membership for Adult = $85 monthly with a Co Payment of = $35
• Individual Membership for Kids = $ 55 monthly with a Co Payment = $35 
• Family Membership Plan A or party of 2 = $ 110 
• Family Membership Plan B or 2 Adults + 1 Kids = $170 monthly with a Co Payment = $35
• Family Membership Plan C or 2 Adults + 2 more = $ 210 monthly with a Co Payment = $35 
• Business Membership Plan = $71 per employee Monthly with a Co Payment = $35 

Signing up to our membership program is as easy as registering on our Membership website, you will be asked to fill in some details about yourself and can have the assistance of one of our many agents to help guide you if you happen to need it.

This will depend if we have a minimum commitment on the membership if there is none, then our members will have an option to choose between monthly or annually billed subscriptions with options to renew the subscription and continue being covered.
